Effective
November 1, 2007, there is a new website available for your use to access
information from Benefit Plan Administration.
You will be able to access Enrollment Forms, Beneficiary Forms,
Withdrawal Applications, Cease Deferral Forms, Hardship Withdrawal
Affidavits, etc. from this site. The
website is: http://www.bpalja.com. Please call BPA or the union hall for the
logon id and password under Client Access. Plan
Enrollment: Enrollment for the 401(k)
plan is held twice every year and commences on January 1 and June 1. If you are a current participant in the
plan and would like to change your hourly deferral amount, you would do so at
enrollment time. Enrollment forms are
due to the 401(k) Plan office two weeks prior to the above dates. You should request an enrollment package
from Teri Schaper at BPA. If you have
questions regarding enrollment, or wish to request an enrollment package, please

make your request in December and May for the enrollment/deferral change
materials.
Ceasing Deferrals: You are able to cease your
401K deferral at any time, but you must submit a request in writing. Please contact Teri Schaper at the 401K
office (mailing address and fax is listed above). Click here
for a Request to Cease Deferral form.
After Teri receives your request, she will send a letter to your
employer’s payroll department informing them of the change and take your name
off of the 401K deferral listing. You
may sign up to begin deferral again at the next enrollment time. Changing
Investment Elections: Effective February 13th,
2006, our retirement plan record keeper has changed to Milliman,
Inc. You will be able to access your
account online or via Voice Response Unit effective February 13th,
2006. The website for Investment
Elections Changes and Fund Reallocations is: http://www.millimanbenefits.com/ or
call the toll-free Voice Response Unit at 1-866-802-6218. Both are available 24 hours a day, 7 days
per week. Customer Service
representatives are available Monday - Friday from 7 a.m. to 7 p.m. (CST). If you have investment
